Pathophysiology of joint pain.

چکیده
The diversity of symptoms experienced by patients with musculoskeletal pain is in stark contrast to the relative paucity of effective treatment. The intensity and character of symptoms vary considerably and may be independent of the underlying disease. In some patients, pain may occur in the absence of demonstrable pathology, whereas in others obviously damaged joints may be relatively symptom free. The past few decades have seen a move away from the concept of a 'fixed' pain pathway towards acceptance of a more flexible system capable of a wide range of responses to varied stimuli. It is now appreciated that tissue injury and inflammation produce prolonged changes in the function and activity of the nervous system. This review will focus first on peripheral events leading to sensitisation and abnormal excitation of sensory nerves, and second on central mechanisms resulting in enhanced transmission in the dorsal horn of the spinal cord and elsewhere. While many aspects of clinical pain remain unclear, the relevance of these mechanisms to symptoms arising as a consequence of musculoskeletal disease will be explored.

Discrete Pathophysiology is Uncommon in Patients with Nonspecific Arm Pain
Background: Nonspecific symptoms are common in all areas of medicine. Patients and caregivers can be frustrated when an illness cannot be reduced to a discrete pathophysiological process that corresponds with the symptoms. We therefore asked the following questions: 1) Which demographic factors and psychological comorbidities are associated with change from an initial diagnosis of nonspecific...
متن کاملNonspecific Arm Pain
Nonspecific activity-related arm pain is characterized by an absence of objective physical findings and symptoms that do not correspond with objective pathophysiology. Arm pain without strict diagnosis is often related to activity, work-related activity in particular, and is often seen in patients with physically demanding work. Psychological factors such as catastrophic thinking, symptoms of...
متن کاملThe Role of Gene Therapy in Cartilage Repair
The key principle of gene delivery to articulations by direct intra-articular injection is to release complementary DNA(cDNA)-encoding medical products that will lead to maintained, endogenous production of the gene products withinthe articulation. In fact, this has been accomplished for both in vivo and ex vivo gene delivery, using several vectors, genes, and cells in some animal models. Some c...
متن کاملCentral sensitivity syndromes: mounting pathophysiologic evidence to link fibromyalgia with other common chronic pain disorders.
The aim of this study was to review emerging data from the fields of nursing, rheumatology, dentistry, gastroenterology, gynecology, neurology, and orthopedics that support or dispute pathophysiologic similarities in pain syndromes studied by each specialty. A literature search was performed through PubMed and Ovid using the terms fibromyalgia, temporomandibular joint disorder, irritable bowel ...
متن کاملThe Interface of Mechanics and Nociception in Joint Pathophysiology: Insights From the Facet and Temporomandibular Joints.
Chronic joint pain is a widespread problem that frequently occurs with aging and trauma. Pain occurs most often in synovial joints, the body's load bearing joints. The mechanical and molecular mechanisms contributing to synovial joint pain are reviewed using two examples, the cervical spinal facet joints and the temporomandibular joint (TMJ). Although much work has focused on the macroscale mec...
متن کاملPosterior Hip Pain in an Athletic Population
CONTEXT Posterior hip pain is a relatively uncommon but increasingly recognized complaint in the orthopaedic community. Patient complaints and presentations are often vague or nonspecific, making diagnosis and subsequent treatment decisions difficult. The purposes of this article are to review the anatomy and pathophysiology related to posterior hip pain in the athletic patient population. EV...
